Immunization Requirements
Proof of the follwing immunizations must be provided:
- Polio - 4 Doses
- DTP (Diphtheria, Tetanus, and Pertussis) - 5 Doses
- MMR (Measles, Mumps, and Rubella):
- 2 Doses* – Kindergarten and 7th grade
- 1 Dose* – Grades 1-6 and 8-12 *On or after 1st birthday
- HEP B (Hepatitis B) - 3 Doses
- for kindergarten enrollment and all 7th grade students
- Varicella (Chicken Pox) – 1 Dose
- Kindergarten and special needs preschoolers
- Out of state entrants grades 1-12
- Physician-documented varicella disease history or immunity meets the varicella requirement
